This form is for anyone going through the process to work with children and/or students for the first time. It will take you through the correct application process based on how you respond to certain questions about yourself and where you would like to serve. 

This form is for anyone age 14-17 interested in serving with children and/or students. Instead of completing a background check, your parent/guardian will need to complete this permission slip for you. 

Do not use this link unless you are specifically told to. The background check information will be collected as part of the Safe Environment Application (see above).
